How to Apply in Three Easy Steps

1. Get the Application Download the application form to the left, or pick one up at a Winding Waters location.

2. Complete the Application

  • Fill out the form completely. Be sure to list every person in your household.

  • Attach proof of income for all working members of your household. A recent pay stub is a perfect example of what to include.

  • Please note: You are not required to include information about your expenses, but it may help us provide discounts to you and your family.

3. Turn In Your Application You have two options to submit your completed application:

  • Drop it off at any of our locations, or
