Community Health Needs Assessment

As part of the requirements for a Federally Qualified Health Center (FQHC), Winding Waters is required to conduct a Community Health Needs Assessment at least every three years. A community health needs assessment (CHNA) is a report that identifies the healthcare and health-related (food, housing, etc.) needs of a community’s residents. The goal of a CHNA is to systematically assess a community’s unmet needs in order to develop strategies to address them.

2022 Report

Winding Waters conducted the 2022 PRC CHNA in partnership with Wallowa Memorial Hospital and Medical Clinics, Building Healthy Families, Northeast Oregon Network, and Wallowa Valley Center for Wellness. You can view the report and analysis below.
